Public Records Request

The Ohio Public Records Act (ORC 149.43) provides the public the right to request access to records from a public office. If you would like to file a public records request, please fill out and submit the form below. You are not required to provide your name, address, email address, or phone number.  However, we would request that you provide some information that would allow us to contact you so that we can ask any pertinent questions and provide the requested records.
